On discharge, Hospital in the Home (HITH) has been organized to visit Henry twice daily to connect the IV antibiotics to a CADD (Continuous Ambulatory Delivery Device) pump for an 12-hour infusion. Henry has a Peripherally Inserted Central Catheter (PICC), which will be removed by HITH after his 10-day course of IV antibiotics. He has four children that live nearby and helps with the house upkeep and cleaning, and delivery of meals when needed. Usually, an independent and self-caring man, this most recent hospital admission has left Henry with ongoing fatigue, significant weight loss, continuous shortness of breath, and pain. Henry works from home as an insurance agent but has taken leave while he recovers.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>An introduction: A brief description of your patient, including their age and diagnosis (not pathophysiology of diseases). An overview of the main points to be covered in the assignment, and a summary (brief) of the 2 (two) SMART goals for your patient and their chronic health condition.<\/li>\n\n\n\n<li>A critical analysis of the self-management framework with relevance and clearly linked to your patient and their health issues.<\/li>\n\n\n\n<li>A critical analysis of interprofessional care with relevance and link to your patient and their health issues. Important: In this section, you must:<\/li>\n\n\n\n<li>Identify 2 (two) other appropriate health professionals for your patient and explain how they will assist your patient in their ongoing chronic disease management.<\/li>\n\n\n\n<li>Identify and discuss 2 (two) key attributes a RN must possess for effective collaboration amongst other health professionals.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">You are to design a self-management plan for your patient that clearly identifies your two SMART goals \u2013 please see below for a format example<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>For each SMART goal, you are to include 3 (three) patient-guided interventions. One (1) of the three interventions for each goal must include the use of the identified health professionals in part 3.
