You can include,\u00a0but it is not limited to, general information on current demographical data and statistics of Pacific ethnic groups living in New Zealand, disease prevalence, life expectancy, and socio-cultural determinants that impact Pacific peoples\u2019 health and well-being.<\/li>\n<\/ul>\n\n\n\n<p><strong>Health Condition, contributing factors and support for Pacific people<\/strong><\/p>\n\n\n\n<p>For your chosen health condition.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Explain the health condition, i.e., disease, prevalence, and impact on Pacific populations. Consider in your explanation what contributing factors influence this health condition.\u00a0 For example, Pacific peoples\u2019 perspectives on health, the impact of westernisation, transition and migration, social and economic factors, systemic barriers, and access to healthcare.\u00a0<\/li>\n\n\n\n<li>Identify ONE national or regional programme\/initiative e.g., Pacific community health care services or National Services with online Pacific health education resources, that addresses this health and wellness issue for Pacific people.<\/li>\n<\/ul>\n\n\n\n<p><strong>Part&nbsp;B &#8211; Health Promotion Project<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Develop a Health Promotion project that you will implement for 6 months. Brainstorm ideas that you think could address the health issue for a specific Pacific ethnic group, age and gender. Be specific about what your initiative is, why and\/what the rationale, the goal and the expected outcome you want to achieve. Discuss how your Health Promotion project could be implemented.<\/li>\n\n\n\n<li>Consider using the Fonofale model of health to develop your project from a Pacific worldview of health and how it could address your chosen health issue.\u00a0<\/li>\n<\/ul>\n\n\n\n<p><strong>For example:&nbsp;<\/strong><\/p>\n\n\n\n<p><strong>Health issue&nbsp;<\/strong>\u2013 Type 2 Diabetes.&nbsp;<\/p>\n\n\n\n<p><strong>Ethnic group<\/strong>&nbsp;\u2013 Tongan.<\/p>\n\n\n\n<p><strong>Target group<\/strong>&nbsp;\u2013 25-39 years old but also consider families.<\/p>\n\n\n\n<p><strong>Health promotion project<\/strong>&nbsp;\u2013 Type 2 Diabetes Prevention.<\/p>\n\n\n\n<p><strong>Rationale<\/strong>&nbsp;\u2013 The prevalence of diabetes is approximately two to three times higher in adults aged 25 \u2013 39 years of Pacific ethnicity compared to those of European ethnicity (Ministry of Health New Zealand. Annual update of key results 2015\/16: New Zealand Health Survey).<\/p>\n\n\n\n<p><strong>The goal<\/strong>&nbsp;\u2013&nbsp;To raise the awareness of Type 2 Diabetes and encourage church members to cook meals and be active.<\/p>\n\n\n\n<p><strong>How<\/strong>&nbsp;\u2013 Work alongside two Tongan churches in Mangere to deliver monthly cooking classes, sports activities and incorporate healthy lifestyle messages to prevent Type 2 Diabetes.<\/p>\n\n\n\n<p><strong>Expected Outcome<\/strong>&nbsp;\u2013 100% of attendees can cook a healthy diabetic meal, have become physically active and can demonstrate their knowledge and awareness of preventing or managing those with existing Type 2 Diabetes better.<\/p>\n\n\n\n<p><strong>The Fonofale model of health.<\/strong><\/p>\n\n\n\n<p><strong>Culture<\/strong>&nbsp;\u2013 Form a project committee of church members aged 25-39 years who can give cultural advice and engage with church members in English and Tongan language.<\/p>\n\n\n\n<p><strong>Foundation<\/strong>&nbsp;\u2013 Include families.<\/p>\n\n\n\n<p><strong>Spiritual<\/strong>&nbsp;\u2013 Deliver the project as a church-based initiative.<\/p>\n\n\n\n<p><strong>Physical&nbsp;<\/strong>\u2013 Consider age-specific activities e.g. sports.<\/p>\n\n\n\n<p><strong>Mental<\/strong>&nbsp;\u2013 Include educational messages on healthy lifestyles and link messages to socio-cultural beliefs of health and wellbeing.<\/p>\n\n\n\n<p><strong>Other<\/strong>&nbsp;\u2013 Include church members 25-39 years, in planning cook classes and activities that are relevant and appropriate for them.<\/p>\n\n\n\n<p><strong>Environment\/Time\/Context<\/strong>&nbsp;\u2013 Run the project in the summertime, in the evenings and partner with other organisations that can provide sponsorship so that the programme is free.&nbsp; Sponsorship for the ingredients for the cooking classes and free healthy food to give away to participants and their families.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Assignment Task Purpose: To identify Pacific worldviews in relation to health and well-being.
