Situation:
Recently admitted to the hospital for a severe respiratory infection. On discharge, Hospital in the Home (HITH) has been organized to visit Henry twice daily to connect the IV antibiotics to a CADD (Continuous Ambulatory Delivery Device) pump for an 12-hour infusion. Henry has a Peripherally Inserted Central Catheter (PICC), which will be removed by HITH after his 10-day course of IV antibiotics. Follow up with his Physiotherapist on discharge to restart regular appointments.

Henry is divorced and lives alone. He has four children that live nearby and helps with the house upkeep and cleaning, and delivery of meals when needed. Usually, an independent and self-caring man, this most recent hospital admission has left Henry with ongoing fatigue, significant weight loss, continuous shortness of breath, and pain. Henry works from home as an insurance agent but has taken leave while he recovers.
