Details
The purpose of this assessment is for students to identify one nursing priority/problem from a chronic care management issue from a case scenario and then design a person-centered nursing intervention plan. The principles of person-centered practice should be considered and integrated into the nursing care plan.
Instructions:
1. Part 1 – Nursing Care Plan
This care plan (table) aims to assist you to:
a) Analyse the information provided in the case scenario,
b) identify one nursing priority/problem (a chronic care management issue), and
c) organise the analysed information using the template provided (i.e.. table) and briefly outline the goals, actions/interventions and evaluation methods.
2. Part 2 – Nursing Care Plan
• Use the principles of person-centered practice to facilitate and formulate a care plan in an essay format. Your care plan should consist of the following components:
a) An introduction: describe the person’s situation
b) Identify one nursing priority/problem (a chronic care management issue) and explain why it is a priority (supported by current literature)
c) Establish two goals – ie. describe what you want to happen, desired outcomes and a time frame)
d) Design a course of action (nursing interventions) with rationales – you should consider different alternatives available and use the current literature to support your actions.
e) Outline one to two evaluation methods – ie. describe how you are going to evaluate the effectiveness of the actions
f) A conclusion

1. Explain the pathophysiological and psychopathological changes occurring within individuals living with a range of chronic health conditions
2. explain the roles and functions of the chronic conditions team, including the individual and family
3. Analyse nursing strategies used in the management of individuals experiencing a range of chronic health conditions, including disability, terminal illness, mental illness, and transition of care
4. explain the role of pain management strategies for individuals with chronic and continuing illness 5. understand the lived experience of an individual with a chronic illness
Your Role and Assessment Task
Ms Jen Bogart had made a full recovery after a stroke in 2018 (as you have seen on the video). However, on the 24th July 2022, she had another stroke and was admitted to an acute care hospital. She was then transferred to a stroke rehabilitation center on 2nd August 2022. You are on your workplace experience placement and have been assigned to look after her with a registered nurse over the next two weeks, starting on 3rd August 2022.
On the next few pages, you will find Jen’s clinical information and relevant clinical charts. You are required to Analyse the information provided including the video on the previous page* using the nursing care plan table/template (Part 1) and then formulate a care plan in an essay format (Part 2) based on her needs and circumstances, ie. person-centered approach.
Background:
Ms Jen Bogart is a high school teacher and she is getting married to her partner in March next year. She had an ischaemic stroke in 2018, aged 32 in which Jen made a full recovery.
Her medical history as follow:
- Ischaemic stroke (cerebrovascular accident) in 2018
- Hypertension – was poorly controlled until after the stroke in 2018
Her current medication:
- Warfarin 5mg PO daily – aims to keep the International Normalised Ration (INR) 2.0-3.0
- Lisinopril 10mg PO daily
