You are required to complete a practical activity with a person to enhance their awareness of care support in the care setting. You should identify a person you are or have worked with. Considering the person's interests, plan a suitable activity that will enhance their experience of care support.

• Observe the chosen person and determine appropriate activity suitable to their needs
• Explain the reasons for your choice of activity
•  Show knowledge of materials / resources that may be needed to carry out activity
• Develop implementation plan
• Show an awareness of health and safety and best practices throughout the planning and carrying out of the activity
• Assignment – Part 2
• Consultation & Communication (LO 2)
• Consultation with the supervisor and person should be evidenced. (3 marks)
• Discuss the the importance of working as part of a multidisciplinary team (2 marks)
• Demonstrate effective communication throughout activity – verbal & non-verbal (2 marks)
• Show an ability to communicate effectively with team members, supervisors or any relevant personnel (2 marks)
