Case Scenario
Hospital Hospital X is a hospital located in a semi-rural area. There are 3 operating theatres and a Stage 1 PACU.
Staff and Roles
RN Jones is a Stage 1 PACU nurse with 5 years of experience working in Hospital X.
RN Lee has been working in Stage 1 PACU at Hospital X for 8 years and is the team leader on shift.
RN Smith is an Anaesthetic nurse who has recently completed a 12-month new graduate programme and has been working as an anaesthetic nurse for 3 months.
Dr Edwards is a Consultant Anaesthetist who works at Hospital X 2 times a week in an elective orthopaedic list that specialises in shoulders, foot and ankles.
Patient
Patient A is a 30-year-old male who was admitted for a day-only elective shoulder arthroscopy and repair of bicep tendon.
Patient B is a 55 year-old- female who has undergone a laparoscopic cholecystectomy who has been in the PACU for 40 minutes receiving analgesia
Patient C is a 4-year-old female who has undergone a tonsillectomy and has been in PACU for 10 minutes.
Setting the Scene
Patient A was scheduled as the first case of the day. He had no significant medical history and was assessed as ASA I. Dr Edwards decided to use an LMA for airway management and administered a balanced general anaesthesia with propofol, fentanyl and sevoflurane. The surgery went smoothly and lasted for about 45 minutes. RN Smith assisted Dr Edwards in the anaesthetic room and monitored the patient during the surgery. The procedure was completed, and Dr Edwards and RN Smith transferred Patient A with the LMA in situ to the Stage 1 PACU.
At this time, RN Jones was providing care to another patient, Patient B who had undergone a laparoscopic cholecystectomy under general anaesthesia. RN Jones has been providing fentanyl pain protocol to Patient B. The other PACU nurse RN Lee is providing care to patient C who is a 4-year-old female who has undergone a tonsillectomy.
