You are dispatched to a patient at a buidling site. On arrival you find a 42-year-old male patient that has fallen off of a building. On initial examination you find the following:
- General impression of the scene – you can see he is unconscious and lying on his side. There is a pool of blood next to his side and it appears he has not moved since he fell. He landed on a concrete floor and his colleauges say that he fell from the 5th story.
- Airway – threatened due to reduced level of consiousness
- Breathing – present, irregular
- Circulation - present, thready
- Disability – clear femur and hip fracture.
You conduct secondary survey and find the following:
- Head – laceration on his frontal lobe, bleeding slowly.
- Face – crepitus felt on his zygoma and his mandible is unstable.
- Pupils – unequal, right pupil slogishly reactive to light, and left eye, briskly reactive.
- Neck – no injuries found, no stepping and trachea central.
- Chest – reduced air entry on the left side, clear on the right. Chest wall unstable.
- Abdomen – rigid
- Pelvis – unstable
- Lower limbs – clear femur fracture right side. Left, stable no injuries.
- Upper limbs – no injuries found
You do vital signs and find the following:
- NIBP – 102/56 mmHg
- HR – 154 bpm
- RR – 25 bpm
- GCS – 6/15 (2 E’s, 1 V, 3 M)
- HGT – 5.2 mmol/l
- CRT - >2 s
You are unable to get a medical history from the patient because he is unconscious, however, his colleagues say that he takes blood pressure medication.
